I don't think "All rights reserved" contradicts the LGPL (or any other free/open) license. As long as a work is under copyright, the copyright owners have an exclusive monopoly over the work and I don't think that the labeling of the work as "all rights reserved" adds to or changes its legal status, it merely informs people that the copyright holder intends to exercise their monopoly and exclude everyone who is not a licensee from doing any of the monopolized actions with that work. The LGPL works within this framework: unless a person accepts the license for an LGPL'd work, that person still has no rights to copy, modify, or distribute the work, thus "all rights reserved." That said, while I am reasonably sure that jpff obtained permission from Maurizio to include his contributions to Csound under the LGPL, I am not sure that anyone obtained his permission to include his documentation for the Python opcodes in the manual under the GFDL. In fact, I have long wondered about the entire legality of Kevin Conder's placing the manual under the GFDL. As far as I know (happy to be corrected), Kevin took the old Public Csound manual, made some changes to it, and unilaterally decided to relicense the entire thing under the GFDL without obtaining the permission of the dozens of people who had contributed to it before him. (Hopefully, not just sticking my foot in my mouth again ...) I did not attempt to contact Maurizio as far as I can tell he did not record any code contribution (looking at Changelog etc).
