I use it to create/modify sound effects for a TV show for young
children. Only for effects that must be timed to picture, or more
commonly, ones of a fantasy nature that don't exist in SFX libraries.
Fairly time consuming so I use it for "special" sounds. However Csound
enforces a certain discipline. I have to create the sound in my mind to
go with the picture, then realize it with Csound. After that comes the
tweaking, until it works well, or well enough. 

This is instead of sitting with a sampler/synth and twiddling knobs for
hours only to find when I'm done it doesn't work with pix. Then that
time is lost. If I do have to start again with Csound, I can save the
orc/sco files for possible later use. Also I can build a library of
instruments to use in similar situations later.

Different sound designers work differently, of course. Csound works for
me.
