Hi,
I'm using Ubuntu with jack and I've no idea how works on Mac, anyway since the commands are quite the same... to have the list of available devices try to run an orchestra with realtime output and have a look at the beginning of the messages in the csound console message (GUI) or in the prompted output (if you are using commad line), with the flag -m135 to have all the messages shown.

This are my settings to connect directly csound output to jack out ports:
-o dac:system:playback_ -+rtaudio=jack
Be sure that jack is using the desired audio sound card (like hd0, hd1... whatever it is)
