Am I correct in guessing that when iflag is 1,
xfreq determines the pitch heard (in a well made soundfont)
but that inotenum will still be used to select the sample
that will be used to create the output?

Or, is it possible that inotenum in this case is ignored,
and the closest base frequency sample is selected and
adjusted to match the desired xfreq?  That's probably
wishful thinking.  If so, how would I go about chosing the
correct inotenum for the pitch that I'm interested in
creating?
