Thanks so much for taking the time to reply. I appreciate the suggestions.

Unfortunately, I don't think either explanation quite fits my situation. The problem isn't that the instrument fails to respond to Note Off messages; rather, subsequent MIDI events don't seem to be sent during the performance.

I'm monitoring Csound's MIDI output using MIDI Monitor, and I can see that Csound sends one initial Note On when the performance starts, but no further Note On messages appear while it is running. When I stop Csound, or the performance ends, MIDI Monitor suddenly shows a burst of Note Off messages.

I've tested this with a minimal CSD as well as my original project. The Terminal output shows that the score is advancing normally and that the notes are scheduled at the expected times, but the MIDI output doesn't follow that schedule.

Everything was working perfectly before I uninstalled and reinstalled Csound. I'm concerned that in the process I may have deleted something that I had previously installed or added myself, and have since forgotten about.

I've also confirmed that I can send MIDI out through the same IAC Driver using DAWs without any problems.

So I'm inclined to think the issue lies somewhere in Csound's MIDI output system, possibly involving PortMIDI or its interaction with macOS CoreMIDI, rather than in the CSD itself.
