This looks odd. Is it possible that somehow two rtpa.dll PortAudio
plugins get loaded (not related to your error, though) ?

There is a known problem with PortMidi that makes it not possible
to open more than one MIDI stream at the same time. This could be
fixed, but until then try using the MME plugin for MIDI input
(enabled by -+rtmidi=mme); note that the device numbers may change.
