Hi All,

I'm not sure who requested it(it was long ago, sorry it took so
long!), but I added a new html-onechunk target to the Makefile in the
Csound manual CVS and this will now generate an index.html that
contains the entire manual in a single HTML file.

There was another request to make a text-only version which I'll try
to get to sometime.
