Hi Oeyvind. I have some C++ VST code here that will create a simple and basic vst from some Csound code similiar to how csLADSPA works. If you want I can send you the MinGW project. It works for VSTi and VST. It was just something one of my students started which I haven't had time to finish. The VSTi stuff borrows heavily from Michaels code, I couldn't get it going on my own. Anyway if you fancy it I can email it to you.
