We can either use a mutex, or process ksmps at a time where that is the
PortAudio buffer size and have Csound driven by the PortAudio callback, or
revert to v18. To implement blocking IO, the PortAudio API obviously uses a
mutex, so we may be able to reuse this code for a Csound mutex instead of
writing a cross-platform mutex from scratch just for Csound.

Which approach do you think best?

As far as I can remember the problem with Portaudio 19 was that it did
not have a working implementation of blocking I/O

Just d/loaded a fresh copy of portaudio_v19 but it does not compile 
(multiply defined functions).  I really was hoping to use v19.
