I would prefer opcodes referenced all in section, each opcode marked with first version supported or removed (since x.x, from x.x to x.x).

The error ended up being a little trickier than what I mentioned.  The
manual entry for printk was linked in twice, once at the top of
csound5/opcodes/top.xml and the other in opcodes/top.xml.  I've
removed it from csound5/opcodes/top.xml.

The csound5 opcodes are now fairly mixed into the main opcodes
section.  I am thinking might as well move all opcodes now into there
from csound5/opcodes. Originally they were separated so that they
would be easier to recognize as being in CS5 only, but these days I'm
more prone to just have it marked in the opcode entry that it was new
in cs5. Any thoughts?
