I have finished a first stage implementation of Paul Nasca's
"padsynth" algorithm as the "padsynth" plugin function table
generator. I have also completed the Csound manual page for it.

I have also improved the ftgenonce opcode, which makes it possible to
encapsulate all function table statements inside instrument
definitions, by enabling the use of named function generators and also
the string parameter for function tables that take it.
