I finally could compile the pdf manual packages using fop-0.20.5.

The crash was caused by the quickref.xml file. Between versions 5.08 and 
5.09 the root element of quickref.xml has been changed from  
to . But in the docbook dtd 
(http://www.docbook.org/tdg/en/html/part.html), a part element must have 
at least one child element among those ones: appendix, chapter, toc, 
lot, index, glossary, bibliography, article, preface, refentry, 
reference. And this is not actually the case in quickref.xml.

I think that the index element would be a good candidate. But then we 
have do rewrite entirely the quickref.py script. I'll try to do this on 
a local copy of the manual to show you the result.

By now, I just manually replaced the  element  in quickref.xml by 
a   element and I compiled the pdf files. The inconvenient is 
that the 'Opcodes Quick Reference' appears as Appendix A instead of Part IV.

When I was debugging the fo file I noticed too that the chuap.png image 
referenced in the chuap.xml file is missing from the images directory.
