I just tested portaudio with ASIO on full-duplex using my MSVC and
it seems to work now. The only caveat is that single channel audio does not
work, but stereo is fine. I will try to sort that one out.

As I predicted, the fix that enabled full-duplex for portaudio on
CoreAudio works here as well. However, at the moment full-duplex
on portaudio winmm/KS/DS will not work, as it uses two separate devices
Will try to fix that too.
