I have updated the Csound manual with some changes to the Csound 5 intro, 
and moved the information on building Csound from the INSTALL file which is 
now deleted, into a new Building section at the end of the manual.

People maintaining builds or installers on other platforms may wish to 
review and/or extend this section.
