I'm working now on a linux box (Ubuntu 8.04) but I kept a small partition with Windows XP, so I could compile the chm manual package. You'll get it here:

I tried to compile the pdf packages and I got the same problems that John described on the 'Fop continued' thread. It doesn't work with fop 0.94 or 0.95 and I get a crash in the end of the compilation with fop 0.20.5. It seems that this crash is due to a null string pointer.
I then tried with xmlroff (the package is available in Ubuntu 8.04) with the following command:
xmlroff -o Csound5.09_manual.pdf --format=pdf Csound5.09_manual.fo
and I got a crash too with the following messages:
(xmlroff:20197): libfo-CRITICAL **: fo-fo-error: root children do not match allowed content
Object path: /FoTree[1]/root[1]
Object path: /FoTree[1]/root[1]
(xmlroff:20197): libfo-CRITICAL **: fo-table-column-error: 'column-width' property must be specified when fixed table layout method is used.
Object path: /FoTree[1]/root[1]/page-sequence[3]/flow[1]/FoBlockBlock[4]/FoBlockBlock[2]/table[1]/table-column[1]
(xmlroff:20197): libfo-CRITICAL **: fo_string_get_value: assertion `string != NULL' failed
Erreur de segmentation
