Oh, I just realized about fop that I wrote some information in the
readme.txt in the manual cvs repository; if you have any problems
installing fop just email back and I can help get that setup (I have
it running on WinXP and Linux and haven't tried on OSX but assume to
work just fine).

As for A4, I took a look at this page:

http://www.sagehill.net/docbookxsl/PrintOutput.html

and found:

xsltproc  --output myfile.fo  \
  --stringparam  paper.type  A4 \
  fo/docbook.xsl  myfile.xml

So if we make a copy of the pdf target in the makefile and modify to add:

--stringparam  paper.type  A4

as part of the call to xsltproc, that *should* get it to work.  I'll
give it a try tonight when I get home from work and if that's fine
I'll commit that.
