Hi All,

I was testing some MIDI things out today and I think i found a bug
with madsr.  When using madsr with a-rate output, it seems to function
correctly, but using madsr with k-rate output, it seems to do the
attack and then decay to zero (no sustain).  I'm looking at the code
but have not yet seen what the problem is.

I've attached a test CSD file.  If you switch to program 2 with your
keyboard to use instrument 2, you will hear the envelope not working.

BTW: I've also just checked in changes to the keyboard.  There is now
a spinner box to change midi channels and programs.
