The common practise is to distribute the instrument code freely, and to credit whoever wrote the instrument you've copied/modified.
There have been no legal licence imposed on Csound instruments AFAIK in the past,
but as the community (us) gains awareness of the legal issues,
it seems to evolve into GPL as the norm for publishing csound instruments.
As earlier, the music you make with those instruments is regarded as a separate work,
not legally connected to the code in any way.

I'm not an expert on these issues,
but this is my impression of how it works.
